1. You can choose whether you want to apply the custom order form before or after the payment. If you want to collect the information before payment tick in the checkbox.
2. To add the custom field to an individual ticket select the ticket name from the drop down list.
3. Click on Add custom field to add a field and give your field a name. Choose whether you want to include it in form and make the field mandatory. Also you can add these fields to all tickets or to selective tickets by checking in the checkbox.
4. Click on save to save the changes.
